POLICIES:

 

BOOKING


Any/all bookings will be made through our site, all bookings will lock you into secured date.

Limited availability, so book to secure your spot.

Leave a valid number/email address for you to be contacted at.

All tattoos will be made in BLACK INK ONLY.

Designs and placement must be emailed to phenomhawaiiarttattoo@gmail.com once paid.

I am not liable/responsible for any misrepresentation.

I have the right to refuse service to any client, in the event- your payment will be refunded in full.

 

ALL APPOINTMENTS WILL REQUIRE :
 A non-refundable $100 deposit is required for all booked services. The deposit secures your spot and the remaining balance is due at the appointment. All sales final, no refunds.

48 HOURS RESCHEDULING POLICY. In the event you would like to reschedule your appointment, you have to text me at (808) 746-5083 within 2 business days. Any reschedules made less than 48 hours will be considered a cancellation/no show and you will forfeit deposit.

LATE POLICY 
Please be courteous of others and be on time for your scheduled appointment. If you are more than 15 minutes late, your appointment will be cancelled and considered a No Show.

PAYMENT POLICY 
The full amount is due at or before your appointment. Payment through our website is the only payment method accepted. No refunds, all sales final.

GUEST POLICY

-Due to limited space 1 to no guest may accompany you, or they may wait in the vehicle outside.
-Children under the age of 13 are not allowed in the studio

 


The following are a few issues that will DISQUALIFY a person for a permanent makeup procedure: 
- Is currently Pregnant or breastfeeding (you’ll be advised to pump and dump after your session)
- Has cancer and are undergoing chemotherapy/radiation
- Steroid medication and prednisone (off for at least 2 months)
- Has an open wound present
- Has aged skin that is extremely thin or severely wrinkled
- Has a bacterial or viral infection

- Have diabetes and under physician care

- Have been in remission from cancer for 1 year
- Have high blood pressure 
- Using blood thinning prescriptions
- Have seborrheic dermatitis

- If you had botox, please wait 6 weeks before booking

- You must be off accutane and retin-a for 4 weeks
